What is a 504 Plan?

At SkyView Academy, our goal is to empower every student to learn, grow, and thrive in a safe, challenging, and supportive environment. A 504 plan is a key part of this commitment. It's a formal document based on Section 504 of the Rehabilitation Act of 1973, a federal law that ensures students with disabilities have access to the same educational opportunities as their peers.

A student may qualify for a 504 plan if they have a physical or mental impairment that substantially limits a major life activity, such as learning, walking, or seeing. The plan outlines specific accommodations that remove barriers to learning and help a student succeed. These accommodations might include things like extended time on tests, preferential seating, or a quiet space to work. The purpose is to provide students with the support they need to fully participate in all school activities.

We are committed to working closely with families to determine a free and appropriate public education (FAPE) for all students. Our team is dedicated to providing students with the resources and accommodations that will best meet their unique needs.

Our 504 Process

For Initial Inquiries: If you would like to learn more or begin the process for a new 504 plan for your student, please fill out our 504 Inquiry Form. This will allow our team to review your request and reach out to you to discuss the next steps.

Triennial Review: A formal, comprehensive review of eligibility will occur every three years. You will be contacted when your student’s review is approaching. 

504 Plan Amendment Process: If you or your child's teacher identify a need for a change or adjustment to the 504 plan between re-eligibility reviews, you can submit a 504 Plan Amendment Request. I will review the request, and minor changes can often be made quickly with a phone call or email confirmation. For more significant adjustments, a brief, focused discussion with essential team members may be scheduled. Please note Amendments will be limited to once per semester to allow observation of the effectiveness of the new accommodations.
